Fixed the location of a parenthetical comment 
The explanation in the parentheses was for the PATH not the QUERY portion. Therefore I moved that explanation next to the mention of PATH. I also added a second parenthetical explanation for the QUERY portion.

Url Dispatch Link is Broken 
Writing an Application document (application.md) refer Url Dispatch document but path is not valid. I fixed with correct value.

Current link navigate to (404)
"./docs/url-dispatch/index.html#using-an-application-prefix-to-compose-applications"

I changed it to
"/docs/url-dispatch/index.html#using-an-application-prefix-to-compose-applications"

Improve extractor examples 
To use `SomeType::extract(&req)` it should be mentioned at least once that `use actix_web::FromRequest` is required. Also extract takes `HttpRequest` by reference.
